1. About these terms
These terms govern the provision of dental and aesthetic services by Forever Dental and Skin (“we”, “us”, “our”) and your use of our website at https://foreverdentalandskin.co.uk. By booking an appointment with us, attending the practice, or using this website, you agree to these terms.
We are a fully private dental practice based at 68 The Street, Ashtead, Surrey KT21 1AW.
2. Booking appointments
Appointments can be booked by phone, email or via our website. When you book, you may be asked to provide identity, contact, medical-history and (for some treatments) payment information. It is your responsibility to ensure the information you give us is accurate and up to date.
A deposit may be required to secure certain appointments — we will tell you in advance if this applies. Deposits are deducted from the cost of treatment.
3. Cancellations, rescheduling and missed appointments
We ask for at least 48 hours’ notice if you need to cancel or reschedule an appointment, so that we can offer the slot to another patient.
- Cancellations made with less than 48 hours’ notice, or appointments missed entirely, may incur a charge equivalent to the cost of the booked time.
- Deposits for cancelled appointments may be retained where insufficient notice is given.
- Repeated late cancellations or missed appointments may result in us declining to offer further appointments.
We will always try to be reasonable in cases of genuine illness, emergency or unavoidable circumstances.
4. Treatment plans, consent and fees
Before any course of treatment we will provide a written treatment plan setting out the proposed treatment, alternatives, risks, benefits and the total estimated fee. Fees quoted are valid for 3 months from the date of the plan unless otherwise stated.
We will not begin a course of treatment until you have given informed, written consent to the plan. You may withdraw consent at any time, but please note:
- treatment that has already been completed must still be paid for;
- laboratory work that has been ordered, custom appliances that have been made, or third-party costs already incurred are non-refundable;
- where a treatment is staged, partial treatment may not deliver the planned result and we will discuss the implications with you.
Our current fee guide is available at the practice on request. Treatment costs are reviewed periodically; long-term plans may be subject to fee adjustments, which we will tell you about in writing before the change takes effect.
5. Payment
Payment is due at the end of each visit unless we have agreed staged payments or finance with you in advance. We accept the following payment methods:
- Debit and credit cards (Visa, Mastercard, American Express)
- Bank transfer (BACS) — available on request for larger treatment plans
- Cash
- Approved third-party finance plans (subject to status — see Finance below)
Invoices issued by Forever Dental and Skin are due within 14 days. Overdue invoices may be subject to interest at 4% above the Bank of England base rate in line with the Late Payment of Commercial Debts (Interest) Act 1998 where applicable. We may pause or refuse further treatment while a balance is outstanding.
6. Finance plans
Where you take out a finance or payment plan with one of our finance partners, the finance is a separate contract between you and the finance provider. Approval is subject to status, affordability checks and the finance provider’s own terms.
7. Refunds
Refunds are considered on a case-by-case basis. Where treatment has been completed satisfactorily and to the agreed standard, refunds are not normally given. Where there is an issue with treatment we have provided, please raise it with us via our Complaints Procedure in the first instance.
8. Guarantees and remakes
Where we offer a guarantee on a specific item of work (for example a crown, veneer, denture or implant), the terms of that guarantee will be confirmed in writing. Guarantees are conditional on:
- you attending recommended check-ups and hygiene appointments;
- you following the care instructions given to you;
- the issue being raised with us promptly; and
- the work not being altered or repaired by another clinician without our knowledge.
9. Use of this website
By using this website you agree that:
- the content is provided for general information about our services and is not a substitute for clinical advice;
- you will not use the website to publish defamatory, unlawful or harmful material, or to attempt to disrupt the website’s operation;
- you will not extract or scrape content for commercial use without our written permission.
All content on this website (text, images, video, design and code) is owned by Forever Dental and Skin or licensed to us, and is protected by copyright and other intellectual-property rights.
10. Links to other sites
Our website may link to third-party sites (e.g. the General Dental Council, the Dental Complaints Service, finance providers). We do not control these sites and are not responsible for their content or privacy practices.
11. Liability
We do not exclude or limit our liability for fraud, fraudulent misrepresentation, death or personal injury caused by our negligence, or any other liability that cannot be excluded or limited by law.
Subject to the above, our liability for losses arising from use of this website (other than the provision of clinical care, which is governed by our clinical duty of care and our professional indemnity arrangements) is limited to losses that are reasonably foreseeable. We are not liable for indirect or consequential losses.

13. Governing law
These terms and any dispute or claim arising out of them are governed by the law of England and Wales, and the courts of England and Wales have exclusive jurisdiction.
14. Infection prevention and control
Patient safety is central to how we run the practice. We comply with the Department of Health's HTM 01-05 essential quality requirements for decontamination in primary dental care, and we apply Universal Precautions with every patient to minimise the risk of cross-infection.
- Instruments are decontaminated to the latest Department of Health guidelines, with single-use items used wherever possible;
- Treatment rooms and equipment are decontaminated between every patient and at the end of each clinical session;
- Items sent to and received from the laboratory are washed and disinfected;
- All clinical team members wear appropriate personal protective equipment (masks, gloves, eye protection and clinical clothing);
- Dental unit waterlines are flushed, disinfected and tested as part of routine legionella control;
- Clinical staff are immunised against Hepatitis B and complete regular training in infection prevention;
- Infection-prevention procedures are audited every six months and reviewed annually.
15. Safeguarding children and adults at risk
We have a legal and ethical duty to protect children and adults at risk from harm, abuse and neglect. The practice has an appointed Safeguarding Lead, and every team member — clinical and non-clinical — is responsible for recognising and acting on safeguarding concerns in line with our safeguarding policy.
- All staff complete safeguarding training appropriate to their role and refresh it regularly;
- Safer-recruitment checks (including DBS checks where applicable) are part of our hiring process;
- Where a child does not attend an appointment we use a “Was Not Brought” approach, considering the child's perspective;
- We work with the local Multi-Agency Safeguarding Hub (MASH), Local Safeguarding Children Partnership and adult social services where concerns arise, and will share information where doing so protects a child or adult at risk from harm.

16. Changes to these terms
We may update these terms from time to time. The version that applies to a particular treatment is the version in force when you book that treatment. The version that applies to your use of this website is the version published at the time you visit it.
